(age 13+)- 2 Comprehensive or Periodic Dental Exams
- 1 Limited / Emergency (problem focused) Exam
- 2 Dental Cleanings
- 2 Fluoride Applications
- All X-Rays
- 15% Savings on Restorative Treatment Needs (no annual maximum or frequency limitations)
- 15% Savings When Seeing Our Contracted Specialty Providers
- 5% Subscription Discount When Family Members Also Sign Up
Annual Periodontal Plan
(required for those diagnosed with Periodontal Disease)- 2 Comprehensive or Periodic Dental Exams
- 1 Limited / Emergency (problem focused) Exam
- 3 Dental Cleanings
- 2 Fluoride Applications
- All X-Rays
- 15% Savings on Restorative Treatment Needs (no annual maximum or frequency limitations)
- 15% Savings When Seeing Our Contracted Specialty Providers

- Laser Bacteria Reduction
Note: These plans cannot be used in conjunction with any insurance plan. By enrolling, you forfeit the ability to access insurance benefits in our office while the plan is active. Should you choose to revert to using insurance benefits before the in-house plan expires, no pro-rated refund will be issued.
